Mindality is a private provider of clinical psychology and mental health services.

Dr Bahar Pourghobad, a Clinical Psychologist and the Founder of Mindality, is the designated Data Controller. Mindality is registered with the Information Commissioner’s Office (ICO) under registration number: ICO:00019715893.

 

3. What Personal Data We Process

We collect and process the following personal data to provide our services:

  • Personal Details: Name, address, date of birth, contact information, payment information, video conference ID (for online sessions), GP contact details, and next-of-kin contact details.

  • Sensitive Information: Signed Therapy Client Agreements, clinical session notes, health history, and any reports provided by other professionals (such as your GP or insurer).

  • Website Information: If you complete our online contact form, we collect the information you provide along with your Internet Protocol (IP) address. We may also use cookies to improve your experience on our website.

 

4. Why We Need Your Data

We use this information to:

  • Provide you with safe, effective, and professional psychological therapy.

  • Manage your appointments and communicate with you effectively.

  • Comply with professional and legal obligations as a Clinical Psychologist.

 

5. How We Store Your Data

We take the security of your information seriously. Your records are kept confidential and stored using secure, encrypted, and GDPR-compliant electronic systems. We retain records only as long as required by professional guidelines (typically 7 years from the date of our last contact). After this period, your data is securely destroyed unless a longer retention period is legally required.

 

6. Who We Might Share Personal Information With

Your information is handled with the utmost care. We will only share your data in the following circumstances:

  • If you are referred by a health insurance provider, or are claiming through a policy to fund your therapy, we will share appointment schedules with that organisation for billing purposes. We may also share information with them to provide required treatment updates.

  • When we need to update your insurance provider or other professionals (such as referrer). We will seek your consent before doing so.

  • If we are required to do so by a court of law.

  • If we believe there is a serious risk of harm to you or someone else.

  • In line with professional standards, clinical cases are discussed with a qualified Clinical Supervisor to ensure high standards of care. This is done anonymously wherever possible.

  • We use trusted providers for essential services like secure data storage and billing. These providers are contractually obligated to keep your data secure and are not permitted to use or share it for any other purpose.

 

7. Your Rights

Under the UK GDPR, you have the following rights regarding your data:

  • Access: Request a copy of the information we hold about you.

  • Correction: Ask us to update inaccurate or incomplete information.

  • Erasure: Request the deletion of your data (subject to legal and professional requirements to retain clinical records for a set period).

  • Restriction/Objection: Limit or object to how we process your data in certain circumstances.

  • Data Portability: Request a transfer of your data to another organisation.
